Primary Policy
Dr. Evans works with individuals, couples, and families via teletherapy to help with multiple life issues. Dr. Evans typically asks all of his clients to explore their basic nature and how their personality contributes to current problems, whether personal, relationship, or family-related. Many people have found they make much more progress in therapy with Dr. Evans than they have experienced in past therapy. Dr. Evans does not try to keep clients in therapy longer than necessary, although he is a big believer in periodic brief therapy checkups for those who feel the need.
Terms and Conditions
Dr. Evans does accept some Colorado insurance. All non-insurance payments are made by credit card. Tele-sessions are $175 for the first session, which typically lasts about 45 minutes to an hour. Subsequent follow-up sessions are $125 and may last up to 30 minutes. There is a $50 charge for cancellations made less than 24 hours prior to the session. Cancellation
Dr. Evans has a 24-hour cancellation policy. To cancel an appointment, just call or text Dr. Evans at least 24 hours in advance, and he will cancel or reschedule your appointment. There is a $50 charge if you cancel less than 24 hours before your session. You may cancel via text, talk, or voicemail: (214) 448-5432. Your agreement to the “Policies” of Log Cabin Counseling gives Dr. John B. Evans, PhD, LCSW permission to charge all fees to the charge card on file. Dr. Evans does accept some Colorado insurance.
Court Appearance
If Dr. Evans is required to prepare for a court appearance or appear in court, you agree to pay Dr. Evans $250.00 per hour for all time involved in any actual or potential court appearance.
